Case management is offered to seniors and others who have disabilities. Such service may include assistance with Medicaid and other applications, help locating appropriate care or care facilities, transportation to medical and social-welfare appointments, and building a “safety net” for vulnerable persons having multiple medical, social or physical problems. Referrals may originate with the person needing help, their relatives, neighbors and friends.
In addition, many people need more limited help such as finding a good person to help in the yard or clean house occasionally. Some of our volunteers can even assess home repairs and safety problems.
